Look no further, join our team today and work from home for part of your day! *_Comfort Home Care is in search of full-time Registered Nurse / Afternoon Visiting Nurse in the North Shore and surrounding areas. _Our nurses visit a diverse population, mostly with mental health diagnoses, performing nursing assessments, communicating with other providers, and maintaining patients in the community as independently as possible.

*Duties and Responsibilities:*

Comfort Home Care is seeking a Massachusetts licensed Registered Nurse / Afternoon Visiting Nurse that can:

* Provides initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
* Initiates the plan of care, necessary revisions, and performs OASIS assessments at appropriate time points when applicable.
* Initiates and performs appropriate nursing interventions.
* Coordinates services, informs the physician and other personnel of changes in the client's condition and needs.
* Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
* Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.

Pay: $65,000.00 - $85,000.00 per year
