$2,000 SIGN ON BONUS!
Seacoast Nursing and Rehabilitation is seeking a -Licensed Social
Worker (LSW) 32 hours per week to join our fantastic team! This is
an exciting opportunity to work at this highly sought-after
BaneCare skilled nursing facility. Come join an amazing team!
Seacoast has a supportive team environment in a beautiful building
located in Gloucester, MA.
Requirements for Licensed Social Worker (LSW):
Must possess a minimum of a B.A in Social Worker, LSW or equivalent
degree with a current social work license.
Nursing home experience preferred
- - - - -Job Duties of License Social Worker (LSW):
- - - Complies social history data
- - - Participates in assessments, development and evolution of
residents care plan.
- - - Responsible for planning and leadership interdisciplinary
care planning weekly meetings.
- - - Documents timely progress notes according to state and
federal regulations.
- - - Coordinates the discharge planning for residents.
- - - Able to interpret and implement established social service
policies, procedures and regulations.
- - - Coordinates services of community agencies for continued
delivery of care upon discharge.
