Manager Location: Bradford,MA Pay Range: $2000/week-$2603/week

skilled RN Case Manager to join our team. The ideal candidate will
be responsible for conducting laboratory tests and procedures,
providing accurate results crucial for patient diagnosis and
treatment. Responsibilities Develop and implement care plans,
coordinating services among healthcare providers, patients, and
families. Act as a liaison between patients and healthcare teams to
ensure patients receive the appropriate level of care and support.
Plan and arrange for safe and effective discharge, including
referrals to home care, rehabilitation, or long-term care
facilities. Monitor and evaluate the use of healthcare resources to
ensure efficiency, cost-effectiveness, and quality of care.
Maintain accurate records and ensure care plans meet regulatory,
legal, and insurance requirements. Qualifications Must hold a
current and unrestricted Registered Nurse license in the practicing
state. Associate Degree in Nursing (ADN) or Bachelor of Science in
Nursing (BSN); BSN is often preferred. Prior experience in case
management, discharge planning, or care coordination is typically
required or highly preferred. Ability to effectively communicate
with patients, families, and interdisciplinary teams while managing
multiple cases. Required: Minimum 1 years experience - required.
Experience in Case Management, LTAC/Rehab - required Active BLS -
