Care / Skilled Nursing Facility experience Description: The
position requires a Registered Nurse in a Long Term Care and
Skilled Nursing Facility setting. The facility accommodates 100
residents, including 96 semi-private and 4 private beds across two
floors. Residents include those with dementia and Alzheimer's, who
wear a tracking device to enhance safety. The electronic medical
record system utilized is a combination of Point Click Care and
paper charting. Staffing ratios are approximately 1 nurse to 17-20
residents. The role may require oversight of the entire facility,
if acting as House Supervisor. Common patient diagnoses include
diabetes, COPD, post-stroke care, heart disease, fractures,
dementia, Alzheimer's, rehabilitation needs, chronic conditions,
and multiple comorbidities. Daily responsibilities include
medication administration, vital signs monitoring, blood sugar
checks, wound care, ostomy management, foley catheter management,
tube feeding administration, and collaboration with team members.
Experience in Long Term Care / Skilled Nursing Facility is
required, with a minimum of one year preferred. New travelers are
welcomed; however, preference is given to those with experience.
Occasional IV antibiotics and possible management of PICC lines may
be required. No tele-monitoring is present in this facility.
Registered Nurses are required to attend daily team conferences. On
every shift, a supervisor is available as a resource. Nurse
Practitioners perform rounds on weekdays, and physicians are
available during specific evenings and twice monthly. Off-hours
support can be obtained through a hospital contact. The
interdisciplinary team includes physical therapy, occupational
therapy, speech-language pathology, case managers, social workers,
and dietitians. Respiratory therapy is not present on-site 24/7,
with nursing staff managing respiratory treatments in accordance
with protocols. A medication dispensing system with bubble packs
for prescriptions is in use, along with a house stock of common
medications. Shifts may vary, with day, evening, and night options
available. The scheduling process is conducted six weeks in
advance, and flexibility is encouraged to fill permanent staffing
gaps. Every other weekend and potential holiday work is required.
There is no call or standby requirement. Floating between floors
may be necessary. The dress code mandates professional appearance,
with specific guidelines regarding scrubs, jewelry, nails, and
hairstyles. Department-specific orientation consists of three
