Physical Therapist Location: Claremont, NH Agency: Ambition 24Hours Pay: $2,648 per week Shift Information: Rotating - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position PT Skilled Nursing/Rehab Claremont, NH (Weekdays, Re post) Role overview A Physical Therapist is needed for a weekday, full time day shift contract at a skilled nursing/post acute rehab facility in Claremont, NH, serving adult and geriatric residents. This 13 week assignment is a reposted order after a cancelled placement and focuses on post acute rehab in a SNF setting. Assignment details Location: Skilled nursing/post acute rehab facility, Claremont, NH 03743 Setting: Skilled Nursing Facility (SNF) / post acute and long term care Position type: Contract, full-time, non permanent Contract length: 13 weeks (02/23/26 05/23/26) Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal break Days: Monday Friday Guaranteed hours: 35 hours per week Gross Blended Rate: $60- $72 p/h Order reason: Re post previous cancelled assignment Key responsibilities Perform comprehensive PT evaluations (strength, ROM, gait, balance, transfers, functional mobility) for adult/geriatric SNF residents. Develop and implement individualized plans of care to improve mobility, independence, and safety. Provide therapeutic exercise, gait/stair training, neuromuscular re education, and balance/coordination interventions. Collaborate with an interdisciplinary team (OT, SLP, nursing, social work, physicians) on care and discharge planning. Document evaluations, daily treatments, progress notes, and discharges accurately in the EMR. Required qualifications (PT) Active New Hampshire PT license (or eligibility before start). Graduate of an accredited PT program. Current BLS/CPR certification. SNF or post acute experience preferred. PTA Skilled Nursing/Rehab Claremont, NH (Weekdays, Re post) Role overview A Physical Therapist Assistant opening exists at the same facility, working under the supervising PT with the same weekday schedule and contract dates. This 13 week reposted assignment supports delivery of PT services to adult and geriatric SNF residents. Assignment details Location: Same skilled nursing/post acute facility, Claremont, NH 03743 Setting: SNF / post acute rehab Position type: Contract, full-time, non permanent Contract length: 13 weeks (02/23/26 05/23/26) Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal break Days: Monday Friday Guaranteed hours: 35 hours per week Gross Blended Pay Rate: $33- $40 p/h Order reason: Re post previous cancelled assignment Key responsibilities Implement PT treatment plans as directed by the PT, including therapeutic exercises, gait and transfer training, and balance activities. Monitor patient progress and communicate changes to the PT. Educate patients and families on exercises, mobility strategies, and safety. Document all treatment sessions and progress in the EMR.
